1. I acknowledge that I understand the nature of and am familiar with the subject matter and practices of fisting, bondage and discipline, sadomasochism, power exchange, fetishism, and other related sexual or dominance and submission activities between or among willing and consenting adults of any sexual orientation or proclivity. I further acknowledge that I do not find viewing or participating in any of these activities, including full or partial nudity, to be offensive or objectionable to me in any manner whatsoever.
   
2. I acknowledge that I am joining a private organization, not open to the public. As a member of such organization, I willingly and with full consent and knowledge waive and relinquish any claim, legal or otherwise, that I might make as to the nature of any allegedly offensive activities such as those listed in Paragraph 1 above or of anything else I might see or experience while attending an activity sponsored by The Slater Society.
   
3. I acknowledge and affirm that, if I am a police officer or other governmental official, I am NOT acting in my official capacity but solely for my own personal interest and enjoyment. If I violate Paragraphs 1-3 hereof, I acknowledge that my activities are for the purpose of entrapping The Slater Society and/or its members, and anything I hear or see will NOT be admissible as evidence in a court of law or any other proceeding.
   
4. I acknowledge that I am at least twenty-one (21) years of age, and that my name as listed below is my true and legal name, not an alias. I agree to produce a valid driver’s license or other valid identification in order to verify my age. Prior to entering any event sponsored by The Slater Society, I understand I may be asked to produce my identification or my membership card, and I agree to produce such identification or membership card.
   
5. As a member of The Slater Society, I acknowledge that in viewing or participating in the presentation of educational material, I may witness actions allegedly considered obscene by statutory language, including full or partial nudity. I affirm and acknowledge that I do not find these acts patently offensive or lewd, nor do they appeal solely to any alleged prurient interest.
   
6. I acknowledge that I will not bring to or consume at any event sponsored by The Slater Society any alcoholic beverage or illegal or illicit substance of any sort, with the sole exception, however, that I may consume alcoholic beverages if the activity is held at or in conjunction with an entity (bar, hotel, etc.) where the sale and consumption of alcoholic beverages are legal and expected.
   
7. I agree to abide by all rules of The Slater Society in effect at the time of my participation in any event sponsored by The Slater Society.
   
8. I acknowledge that I will never solicit for, negotiate, or consent to any form of monetary payment or other valuable consideration, in exchange for sex during any activity sponsored by The Slater Society. I am aware that such conduct is illegal under Illinois state statutes and other city and local ordinances.
   
9. I assume all risks, including injury, incident to my attendance at and participation in events sponsored by The Slater Society. On behalf of myself, my heirs, estate, assigns, legal guardian, and personal representatives, I agree to indemnify and hold harmless The Slater Society, its officers, directors, organizing committee, volunteer staff, and members, as well as any designated host establishment(s) where events sponsored by The Slater Society may be held, including the owners, agents, successors and assigns of such host establishment(s), from any claim of liability that may arise out of my attendance at and participation in events sponsored by The Slater Society.
   
10. I agree that I will not bring any recording device of any kind or nature whatsoever, including but not limited to video and/or audio, to any event sponsored by The Slater Society or, if applicable, at a space sponsored by The Slater Society, such as seminars, workshops, demonstrations, or dungeon play space. I further acknowledge and agree that should any such recording device be brought to an event sponsored by The Slater Society by me or by someone acting on my behalf, such equipment will be immediately confiscated, and all film, media, or recorded data will be immediately and irrevocably destroyed.
    
11. I acknowledge that if I violate any term or condition of this Membership Agreement and Waiver, I may be subject to expulsion from any events sponsored by The Slater Society and may forfeit my membership in The Slater Society.
    
12. If any provision of this Membership Agreement and Waiver is held to be invalid or unenforceable, it does not invalidate the entire Membership Agreement and Waiver; all other provisions shall remain in full force and effect. This Membership Agreement shall be construed under the laws of the State of Illinois, and any claims must be brought in the Circuit Court of Cook County.
